What’s a sickness self-certification form and when do you need one?
This is a straightforward form for your employees to complete when they return to work from a period of absence caused by sickness. This form should be completed if the absence was for fewer than 7 calendar days.
If the sickness absence was for longer than that, the employee should provide a Fit Note from their GP/other doctor, which will contain detail of the sickness and the doctor’s view on the employee’s ability to return to work – and it must be completed by the doctor, not the employee.
The employee should be clear about where you will keep this completed form. It should be kept with their records.
If you intend to keep a record anywhere else, such as in part of an HR software solution that your business uses, you should also make that clear here too.
